Leaking injector the most likely culprit. May never do it again, but I'd check them all the same.

Just weld up the manifold. In an explosion like that, the pressure will blow the weakest area, in this case the welds. It has to go somewhere. I've seen a Suzuki running nitrous (leaking fuel solenoid) blow a hole in the plastic intake runner with a similar massive explosion. Bit of gaffer tape over the hole & it carried on for a few more runs.
