List what you have to eat on a typical DAY and list what you do for exercise for the WEEK.
Also list what you mess up with on your nutrition ( booze, crap food etc ) and how many times you do this a WEEK.
I'm assuming since you are a diabetic you must follow some sort of nutritional regime or you will be fucked up so you maybe half way there
