Could be too many things really. Given that it can't shift into 4th on the downshift, I'd get my local mechanic to have a look, just to make sure something isn't broken and about to break some more things in the box.
Can you rev match when downshifting and get it into gear?
If it just grinds slightly, you can use redline oil to hide the noise for a while.
