The flywheel could be faulty. We purchased one froma wreckers yard once that had been in a major frontal accident. (they didn't mention that) When it was machined prior to installation the centre hub was bent enough that it caused uneven clamping.

Can you measure the step height on the flywheel around all the mounting points to rule that out as well? Was the flywheel machined at all?
