Not much really...

From my experience I have found that you need:

To enable AL:
Fusion ECU (the M.R. Fusion is one of the better ones)

To handle the small blasts:
Engine needs to have the flux capacitor upgraded.
All lines need to be of fallopian tube quality.
For added safety the fuses should be changed over to T.I.M.E. Circuits.

For tuning:
No one is better than Emmett Brown for tuning this system together.

Well, to say cars have had engine failures soley due to antilag is probably a bit misleading, but they certainly go hand in hand
from the top of my head, joesac, protorob, wrp10 adams car, ja50n, sti tuned, wrx-sti brad, clints old car etc all have a: used antilag and b: needed a rebuild. sure if it doesnt kill it straight away but it is detrimental to the life of the engine
