i played with squid quite a bit back in my day...tried to block ppl streaming video/music as it chewed up our quota. Didn't have any luck back then...can you block everything and only allow certain sites?
block port 80 - http and 443 - https = no problems

Block outgoing requests except anything but your squid proxy? are they changing proxy server or going 'through' a website?
edit: don't forget about things like 'Tor' which people can use to skip proxies too. It's a very hard thing to stop PZ, you need to have quite a controlled environment.
