Default hub issues

any ideas as to what would be causing my wheel nuts to come loose?
Car had a knock narrowed it down to wheel nuts being loose, tightened them up went for a very short drive, literally 200m and started to knock again...

Wheel nuts were loose again, a bolt has now sheered off the hub, leaving 4 remaining.
Car has been in my garage for a month after i heard this knocking sound,i looked at everything and cant see anything out of order besides the wheel nuts being loose.

Cars being flatbedded to mms tomorrow for other work im just stumped as to what would cause the wheel nuts to loosen in 200m of slow steady driving? Ideas please gents.
