Default Clutch Shudder

What causes clutch shudder? I have to take off really slow, if i try to give a bit of a launch it feels like the car is going to rattle to death. Sounds and feels bloody terrible. I dunno what clutch is in it, but it's not stock.

* cold clutch
* new clutch - needs "wear" in
* flywheel needs machining

Yeah I looked at that. but mines not new, and does it every take off all day, so its not a cold thing. And it still grips like usual between shifts. So reckon i should just get a new one...
