who installed the rear diff? have you had an alignment recently?

You havn't answered if it has DCCD or not, if it does then there's your answer! Thats the whole reason i got DCCD

I installed it, it has the correct shafts. I can't recall exactly the circumstances but I believe the original rear LSD was reassembled with 3.9 ratio gearing.
I'd be looking at the tyres, tyre pressures, whether any sway bar links are broken and whether there's excessive toe out on the rear?

Was going to suggest tyres myself. What brand of tyre? Maxis are like ice skates in the wet, their on the back of mine till I get kumos like I have on the front.

Was it a bit of a random route or the sort of route a truck that was spilling diesel or oil might take???

It's unbelievable how slippery things can get with just a little lubrication!!!

+1 on checking for anything loose or broken in everything suspension related.
